Northwestern Illinois Continuum of Care

The Northwestern Illinois Continuum of Care (The Continuum) works to prevent and end the devastation of homelessness. Participation is open to all those who have an interest in helping others. Membership in The Continuum includes government entities, elected officials, non profit organizations, financial institutions, businesses, housing developers, churches, health care providers, educators, homeless and formerly homeless persons.

Region 1: Knox
Region 2: Henry, Mercer, Rock Island
Region 3: Carroll, Lee, Ogle, Whiteside
Region 4: Bureau, LaSalle, Marshall, Putnam, Stark
Region 5: Jo Daviess, Stephenson

Continuum Activities:

  • Develop a 10-year plan to end homelessness
  • Implement a Homeless Management Information System (HMIS) to better track and aggregate characteristics and service outcomes
  • Bring more awareness to the issue of Homelessness and the lack of Affordable Housing
  • Evaluate and expand housing, services, and programs for individuals and families experiencing homelessness
  • Survey housing providers, conduct street counts, and compile data of available emergency beds, housing units, services, and the resulting gaps
